What to do if a student or staff member is symptomatic (English)

If a staff member or student has had symptoms of COVID-19, the staff member or a parent/guardian for the student must attest, either verbally or by completing an attestation form, that the individual has met all of the criteria for returning to school. This is a sample attestation form that schools and parents may use. It does not need to be filled out by a healthcare provider. Please refer to the Pre K-12 Playbook for more details.
